O Diabetes Mellitus (DM) é uma doença crônica de alta prevalência mundial e com estimativas de aumento preocupante da incidência. As complicações decorrentes da doença são muitas vezes incapacitantes, devido às amputações, e demandam um alto custo para os serviços de saúde pela recorrência e necessidade de hospitalizações prolongadas. As úlceras crônicas de membros inferiores são umas das principais complicações e necessitam de tratamento adequado na atenção básica de saúde. Plantas medicinais e fitoterápicos são alternativas de tratamento. A seiva de C. lechleri, extraída da planta conhecida como Sangue de Dragão, tem demonstrado um grande potencial para o tratamento de feridas devido à sua ação cicatrizante, além de apresentar propriedades anti-inflamatórias, antitumorais, antimicrobianas, entre outras. O objetivo deste estudo foi acompanhar a evolução de úlceras crônicas nos membros inferiores de pessoas com diabetes, cadastradas no SIAB de Lajeado – RS e submetidas a tratamento de três meses com pomada à base de seiva de C. lechleri. Trata-se de um estudo de campo, experimental com abordagem quantitativa, cuja amostra foi composta por quatro pessoas com úlcera crônica de membro inferior devido ao diabetes que não responderam adequadamente aos tratamentos empregados até o início do experimento. Os dados foram coletados por meio de registro fotográfico, exames laboratoriais e um instrumento semiestruturado, no qual foi realizada a caracterização da amostra, avaliação dos pés e da ferida. Foram realizados curativos diários, precedidos de limpeza com SF morno em jato. Para a análise dos dados foi utilizado estatística descritiva e os resultados descritos apresentados utilizando média (desvio padrão). As quatro participantes tinham agravantes para a cicatrização de suas feridas: diabetes, obesidade, uso de medicações, contaminação, tratamentos inadequados. Duas participantes apresentavam perda de sensibilidade nos pés. Observou-se uma redução do tamanho médio das feridas de 123,86 (127,69) cm² para 99,74 (99,33) cm², no entanto esta diminuição não foi estatisticamente significativa (t= 1.5573; p=0.2172). A pomada, com 10% de seiva de C. lechleri não apresentou efeito antimicrobiano. São necessários mais estudos com esta seiva para determinar qual a melhor formulação a ser utilizada no tratamento de feridas.

The complications of the disease are often disabling due to amputations, and require a high cost to health services by the recurrence and the need for prolonged hospitalization. Chronic lower-limb ulcers are one of the main complications and require appropriate treatment in the primary health care. Medicinal plants and phytotherapics are alternatives of treatment. The sap of Croton lechleri, known as Dragon’s Blood, has shown a great potential in the treatment of wounds due to its healing action, besides to show anti-inflammatory, antitumor, antimicrobial and others properties. The aim of this study was to follow up the evolution of chronic lower-limb ulcers in people with diabetes, registered in SIAB Lajeado/RS/Brazil and subjected to a treatment of three months with ointment based on Croton lechleri sap. Field studies, experimental, with a quantitative approach, whose sample was composed of four people with chronic lower-limb ulcers due diabetes that did not adequately responsive to treatments employees up to the beginning of the experiment. The data were collected by photographic record, laboratory exams and a semi-structured instrument, where was held the sample characterization and the evaluation of the feet and the wound. Daily dressings were performed , preceded by cleaning with warm saline solution in a jet. For data analysis, descriptive statistics was used, and the described results presented using mean (standard deviation). The four participants had aggravating factors for the cicatrization of their wounds: diabetes, obesity, medication use, contamination, inadequate treatment. Two participants had loss of sensation in the feet. It was observed a reduction in the average size of the wounds of 123.86 (127.69) to 99.74 cm² (99, 33) cm², however this decrease was not statistically significant (t= 1.5573; p=0.2172).
